Box 514 Maseru 100 Lesotho Tel: (+266) 27323277 03rd August 2023 RE: REQUEST FOR QUOTATIONS (RFQ) FOR PROCUREMENT OF HARDWARE FOR FUNCTIONAL LMIS RFQ No. LS-MOH-269297-GO-RFQ-LNHSS-2022. • The Government of Lesotho has received a credit from The World Bank towards the costs of financing Southern Africa Tuberculosis and Health System Support implemented by the Ministry of Health.

It is intended to apply part of the proceeds of this credit for payment for the supply and delivery of Hardware for functional LMIS. • You are invited to submit your most competitive quotation for Items as per Table 1 | Item | Description | Quantity | Delivery Place | Delivery period | 1 | Tablets | 185 | Ministry of Health HQ | 3-4weeks • Bid Price: The prices shall be quoted in Maloti. Bidders must quote for all items in lots as evaluation will comprise all items in lot. • Quotation Validity: Quotations shall remain valid for a period not less than 30 days after the deadline date specified for submission. • Delivery Period: goods or services must be delivered within the period stated in Table 1 on line 2 • Attachments: Quotations must be accompanied by valid copies of a valid trader’s license and certified copies of a VAT clearance certificate. • Selection Criteria: lowest evaluated and compliance with the required services. • Penalties: If the Contractor awarded the contract to provide services under this order fails to deliver or complete all delivery within the period stated in the quotation, a penalty will be charged at 0.5% per week up to a maximum of 3% at which the order will be canceled in writing by the Purchaser. • Each bidder shall submit only one quotation.

Sealed quotation shall be submitted in writing at the address, before the date and time mentioned in paragraph 15 below. • Bidders should note that alternative bids/quotations will not be accepted. • Award of contract: The Client will award the contract to the bidder whose quotation has been determined to be substantially responsive and who has offered the lowest evaluated quotation price for lot 1. • Notwithstanding the above, the Client reserves the right to accept or reject any quotations and to cancel the bidding process at any time prior to the award of the contract. • The bidder should specify the warranty/guarantee period, otherwise the normal commercial warranty/guarantee shall be applicable to the supplied goods. • You are requested to provide your offer latest by 12:00 hours (local time) on 11th August 2023.
