HEALTH SYSTEMS STRENGTHENING FOR HUMAN CAPITAL DEVELOPMENT PROJECT JOB -ADVERTISEMENT REQUEST FOR EXPRESSIONS OF INTEREST Title of Assignment: Position of Financial Management Specialist Reference Number: SZ-MOH-455103-CS-INDV • Background The Government of Eswatini (GOE) through the Ministry of Health (MOH) is undertaking the COVID-19 Emergency Response Project (Eswatini COVID-19 ERP; P173883) and Health System Strengthening for Human Capital Development in Eswatini Project (HSS4HCDP; P168564) with support from the World Bank. • Project Context To meet the National Health Sector Strategic Plan’s (NHSSP II 2019-23) policy objective – “build an efficient, equitable, client-centered health system for accelerated attainment of the highest standard of health for all people in Eswatini” – the MOH recognizes the need to invest in health system strengthening to improve the coverage of essential services and the quality of care. Modernizing the foundations of the health system and applying evidence-based improvements are key to achieving better quality care and, through that, reduced child and adult mortality and stunting for human capital formation. To address key sector challenges in the health sector, the MOH is implementing the HSS4HCDP with the aim of improving the coverage and quality of key reproductive, maternal, neonatal, child and adolescent health (RMNCAH), nutrition and non-communicable disease (NCD) services (hypertension and diabetes) in Eswatini. The project focuses on strengthening the health system and ramping up investments in RMNCAH, nutrition and NCD services to address critical human capital challenges, including stunting and child and adult mortality, applying a life course approach. The project includes the following components: Component 1: Improve health service delivery to increase the coverage and quality of health services to build human capital; Component 2. Increase community demand for RMNCAH, nutrition and NCD services; Component 3. Strengthen the MOH’s stewardship capacity to manage essential health and nutrition services and project activities; and Component 4: Contingent Emergency Response Component. The project is complementary to the Eswatini COVID-19 ERP and is designed to deepen and extend its investment impact through focusing on medium-term structural changes and the modernization of the health sector. • OBJECTIVES: To maintain appropriate financial management responsibilities for the two projects (HSS4HCDP - P168564 and the COVID-19 Emergency Response Project - P173883). In particular to ensure that: (a) funds for the Projects are used efficiently and for their intended purposes, and (b) all financial requirements and obligations specified in the Government regulations, rules and guidelines and the World Bank Financing agreements and Disbursement and Financial Information letter (DFIL) requirements are observed. • DUTIES AND ACCOUNTABILITIES: Specific responsibilities, working in collaboration with the Project Accountant and Assistant Project Accountant, will include, but will not be limited to: Programming, Planning & Budgeting: • Assist in the development of the project work plan and budget. • Monitor and review the work program, budget, (e.g., monitoring of projects), chargeback, etc., identify issues, and discuss them with the PIU. • Monitor implementation of and compliance with procedures and systems for financial management. • Provide information and analyses with respect to the management of project resources (e.g., budget, work program, unit costs/performance measures, etc). • Collect budgetary information required as inputs for sectoral quarterly reviews (including collecting, integrating, and analyzing data , as relevant). • Contribute to capacity building of MOH staff in Financial Management Financial Control, Reporting and Communication: • Maintain internal controls by monitoring expenses, revenues, and administrative transactions to ensure that all project-related activities, adhere to the applicable Government and World Bank policies and procedures. • Maintain adequate financial management arrangements to support the deployment of project resources in an economic, efficient, and effective manner to achieve the desired objective. • Monitor and review the work program and identifying issues and preparing reports for management as needed. • Manage projects cash flows and ensure timely submission of disbursement requests to IBRD for replenishment of the Loan proceeds. • Review and approving monthly Bank Reconciliation Statements – to be prepared by the Assistant Project Accountant. • Proactively establish and maintain interface with internal technical unit clients, including the provision of analysis to Project Coordinators, PIU and Technical Leads on the management of resources, as well as participate in the meetings with the unit management, as needed. • Advise staff in technical units to improve knowledge, awareness and compliance with respect to budget policies and internal control procedures. • Coordinate the preparation of quarterly interim financial reports and annual financial statements and ensure timely submission to relevant stakeholders. • Provide inputs to internal and external progress reporting and monitoring of the program in accordance with the terms of donor agreements and Bank policies. • Liaise with the internal/external auditors, prepare for audits, and respond to audit team as required. • Assist with any other relevant duties assigned. • Add the role of supervising Assistant Project Accountant and how to engage with the Senior Accountant. • QUALIFICATIONS • Bachelor’s degree in finance, accounting, business, economics, or commerce. • A professional accounting qualifications (CA, CPA, ACCA, CIMA). • At least five (5) years of experience in accounting and financial management and must have served articles. • Preference will be given to candidates with managing donor funding • Relevant knowledge of International Public Sector Accounting Standards (IPSAS) and the International Financial Reporting Standards (IFRS). • Knowledge of the application of accounting, auditing and financial reporting systems and software packages; • Knowledge and experience of the World Bank's financial management arrangements will be a preference. • Knowledge of/and skills in applying work programming, budgeting and financial management tools and techniques to support effective and efficient resource management and business planning. • Work experience in preparing and monitoring budgets, as well as experience in monitoring expenses relative to planned allocations. • Ability to research, analyze and synthesize information, interpret data, retrieve information from computerized accounting systems, and to summarize results clearly and concisely. • Capacity to function as a member of a diverse and multi-disciplinary team. • Strong quantitative skills and attention to details. • Strong communication skills with ability to prepare presentations and discuss findings in written and oral form. MAIN RESPONSIBILITY To ensure timely implementation of project monitoring and evaluation (M&E) activities including the planning of data collection activities, managing project progress reviews, and reporting on progress including updating all indicators in the results framework (RF). The Senior M&E Officer will work closely with internal and external stakeholders in coordinating and supporting all data collection and reporting arrangements on project progress. DUTIES AND ACCOUNTABILITIES Specifically, under the supervision and guidance of the Project Coordinator, the Senior M&E Officer will perform the following duties and responsibilities: Project M&E: • Update/review project-level M&E plan, system and associated work plans for each component/activity (as reflected in the RF); • Review and update, as needed, M&E system/mechanism, process and procedures of the project, and forms and formats for project activities under its various components; • Update/refine an M&E database for the projects; • Lead evaluations and assessments related to the overall project or project activities • Attend, participate and provide necessary inputs for preparation of and during project meeting, workshops and trainings; • Serve as focal point for providing M&E inputs on Implementation Status Reports; • Prepare consolidate progress reports for project management to submit to the relevant stakeholders, in accordance with approved reporting formats and schedule; • Coordinate the preparation and consolidation of annual work plans, as well as monthly and quarterly project reports and other deliverables; • Review monitoring reports, analyze them for impact evaluation and to identify the causes of potential bottlenecks in project implementation and make recommendations; • Ensure timely data compilation and M&E reporting for project implementation and completion reports, including the semi-annual Implementation Status Reports, annual reports, and the final Implementation Completion and Results Report (ICRR) M&E technical assistance to the technical teams implementing the project activities and other PIU members: • Develop TORs for M&E tasks to be carried out by consulting firms (e.g. the design of surveys, evaluations (mid-term and final evaluation), etc. using a combination of quantitative and qualitative methods; • Ensure quality control of M&E outputs (e.g. surveys etc.), including by contributing substantively to the design and field testing of the monitoring methodology, supervising design and implementation of the survey, participatory data collection methods and protocols, data verification techniques, and other technical evaluation and analytical tasks conducted; • Support technical work such as reviewing M&E methods, carrying out training needs assessment, designing M&E tools, and providing M&E technical assistance to the PIU and technical teams. • Ensure mentoring and training of project team to build M&E capacity. • Monitor project progress through field visit as may be required, offer feedback and keep regular communication with related field and central level staff including World Bank project team. • Support bi-annual implementation support missions commissioned by the World Bank on the project by providing project related information. REPORTING: The Senior M&E Officer will report directly to the Project Coordinators and ensure timely reporting for project implementation and completion. The Senior M&E Consultant will closely work with the Sr. M&E Officer. QUALIFICATIONS & EXPERIENCE: • Post-graduate degree in public health, social sciences, statistics, (health) economics, data science. • Eight years relevant work experience in M&E five of which must have been at a managerial level within government, international NGO, or development partners. Experience working in the health sector would be a plus; • Experience in the design and/or implementation of an M&E system, preferably in the health/public sector; • Experience in conducting training and facilitation of M&E activities. • Proven experience of conducting socio-economic research and program and project performance assessment. • Advanced and applied knowledge in data analytics using data analytical software • Ability to write clearly and concisely, and have sound quantitative skills (managing, analyzing and interpreting data). • Demonstrated experience in M&E of development project activities. • Innovative and able to apply technology-based platforms, and adopt changing methods • Broad knowledge and strategic use of data for evaluation and performance assessments; • Very strong planning, system development and organizational skills including results-based management. DURATION OF SERVICE The Contract will be for a duration of 14 months from August 2024 to 30 September 2025. HEALTH SYSTEMS STRENGTHENING FOR HUMAN CAPITAL DEVELOPMENT PROJECT JOB -ADVERTISEMENT ADDENDUM (EXTENSION OF CLOSING DATE) REQUEST FOR EXPRESSIONS OF INTEREST Title of Assignment: Consultant to Conduct and Coordinate Micronutrient Deficiency Assessment Reference Number: SZ-MOH-431101-CS-INDV • Project Background The Government of the Kingdom of Eswatini through the Ministry of Health (MOH) is implementing a project that endeavours to improve the coverage and quality of key reproductive, maternal, neonatal, child and adolescent health (RMNCAH), nutrition and non-communicable disease (NCD) services (hypertension and diabetes) in Eswatini. The 5-year project entitled “Health System Strengthening for Human Capital Development (HSSHCD) Project” under the MOH will make substantial investments in strengthening the health system and improving the nutrition service delivery across the life cycle in order to address critical human capital challenges specifically stunting and child and adult mortality. The project has four components namely, Component 1: Improve health service delivery to increase the coverage and quality of health services to build human capital; Component 2: Increase community demand for RMNCAH, nutrition and NCD services; Component 3: Strengthen the MOH’s stewardship capacity to manage essential health and nutrition services and project activities; and Component 4: Contingent Emergency Response. • Background to the Assignment Micronutrient deficiencies cause a variety of morbidities and increased mortality which are most severe in children, adolescent girls and pregnant women. Micronutrient surveys help to assess micronutrient deficiencies with the largest public health burdens amongst women of reproductive age, children 6-59 months, and adolescents to inform policy and program scale-up. The only nationwide survey in Eswatini on micronutrient deficiency in children was the Demographic and Health Survey (DHS) of 2007, which estimated iron and vitamin A consumption and the prevalence of anemia and vitamin A deficiency in the 4 regions of the country. Prevalence of anemia was 42% in children 6-59 months and 30% in women 15-49 years of age. The survey will therefore provide updated information in this regard. Objectives of the study • To determine the prevalence of vitamin A deficiency, vitamin D deficiency, iron deficiency anaemia, vitamin B12 deficiency, folate deficiency, zinc and iodine deficiency in children 6-59 months, school-age-children, adolescents, and women of reproductive age in Eswatini. • To determine the risk factors for micronutrient deficiencies in children 6-59 months, adolescents, and women of reproductive age in Eswatini. • To recommend evidence-based interventions and policies to address micronutrient deficiencies in Eswatini including a micronutrient provision program in country. The target group living in rural and urban areas of the four regions of the country is: • Children 6-59 months of age • School-age children (5-9 years) • Adolescents (10-19) years • Women of reproductive age (15-49) years. • Purpose of the assignment The purpose of this assignment is to support the MOH in planning, designing and conducting a micronutrient specific survey in Eswatini, to generate data and evidence (at national and regional levels) on the micronutrient and nutritional status of children, adolescents and women in the country to inform policy development and programming. • Scope of Work for the Assignment • To develop an inception report detailing the survey protocol and timeline • To develop and finalise the standard operating procedures, training manual and all relevant study documents. • To train and supervise data collection teams and ensure adherence to all study procedures. • To lead, oversee, and guide the survey implementation including data collection, quality assurance and coordinate laboratory analysis with the Head of the Nutrition, who will be responsible for laboratory data analysis. • To provide study progress updates to relevant stakeholders, including the advisory or technical working groups. • To analyse survey data and generate (draft and final) reports. • To package and present study findings to the core team, senior management team, and relevant stakeholders. • Competencies and experience required • Masters in food and nutrition, Public Health, Epidemiology, Biostatistics, Research and Evaluation, or Medicine and other related fields; • Minimum 10 years of practical experience in designing, coordinating and managing large population health research surveys (e.g. DHS, MICS); Experience of conducting/managing a large micronutrient survey is preferred. • Proven experience in developing and leading household and clinical surveys, proven experience of conducting micronutrient surveys and producing the final reports. • Must be familiar with a Statistical Package suitable for nutrition related data (e.g., STATA, R-Studio etc), data collection and management (e.g RedCap or Kobo collect) and all relevant computer applications in general • Strong analytical, writing and presentation skills. • Working experience in Southern African countries is preferred. • Strong analytical, report writing skills, presentation skills and ability to meet deadlines. • Excellent spoken and written communication and facilitation skills in English. The project is complementary to the Eswatini COVID-19 ERP and is designed to deepen and extend its investment impact through focusing on medium-term structural changes and the modernization of the health sector. • Scope of the Assignment The Ministry has a fully-fledged Biomedical Engineering Department with an inadequate complement of Biomed Engineers. Currently there are two (2) Biomed Engineers in the Ministry of Health. There is therefore a need for additional Biomed Engineer to support the Project activities, Strengthen medical equipment management and maintenance within the Unit and the Biomed offices who are responsible for the whole facilities under the Ministry. • Objectives of Assignment The objectives of the assignment are to: (a) review medical equipment technical specifications, (b) evaluate medical procurement activities, (c) supervise the installation of medical and non-medical equipment; and (d) further train technical staff and end users all health facilities in Eswatini. • Duties and Responsibilities Under the overall guidance and direct supervision of the Senior Biomedical Engineer the officer will execute the following tasks: • Review medical equipment technical specifications provided by the end beneficiaries and recommend most appropriate technical specification, as needed; • Evaluate procurement activities related to medical and non-medical equipment provisions; • Execute field visits to supervise the installation, maintenance and repair of medical and non-medical equipment, and ensure that the equipment is delivered as per the approved technical specifications and current industry standards; • Develop preventative maintenance schedules and guidelines that are integrated with hospital SOPs, and train all technical staff on proper testing and other procedures to be followed when performing scheduled maintenance; • Ensure newly procured equipment are captured by technicians are recorded to the asset management system for all health facilities; • Attend regular Regional Health Management Team meetings for joint problem solving and for scheduling monthly and annual work activities; • Conduct an equipment needs assessment, highlighting the new equipment needed, its specifications, comparative analysis of different types/brands and guidance to the procuring unit. • Provide technical guidance to technicians in all health facilities on maintenance and repair of equipment(s), supervise periodic calibration, lead new equipment acquisition and recommendations, evaluate the safety, efficiency, and effectiveness of biomedical equipment and its use to ensure high quality service delivery; • Help to draft annual budget of the department; and • Make recommendations for systems improvements and arrange for equipment disposal if needed. • Qualifications required I- Academic Qualifications: • Bachelor of Science degree in Biomedical /Electrical &Electronic Engineering emphasis on Biomedical or relevant field. II- Years of Experience: • Relevant experience of not less than 3 years in operational experience. III- Competencies: a. Strengthen the MoH’s stewardship capacity to manage essential health and nutrition services and project activities and Component 4: Contingent Emergency Response Component. • BACKGROUND TO THE ASSIGNMENT The MoH operates under the direction of his Majesty’s Government. The Ministry has the core objective of improving the health of the people of Eswatini by providing leadership in the production, delivery and utilization of health services which will consistently increase longevity and quality of life. The Ministry through Central Medical Stores (CMS) is responsible for the supply chain management of all health commodities including medicines in the public sector. In Eswatini, the CMS is the only depot responsible for supplying all the government health facilities including faith-based/mission facilities and some private facilities with medicines and medical supplies.[1] CMS supports the mission of the MoH by providing preventative, curative and diagnostic medicines that are of acceptable quality, safe, and effective. The main objective of CMS is to ensure regular and uninterrupted supply of quality medicines and medical supplies to health facilities in Eswatini, ensuring that the EmaSwati population has equitable access to these commodities. To deliver this mandate, the Ministry aims to strengthen the capacity of its operations, procurement, and supply chain management in its key institution. This policy directed all Ministries and Government Departments, state owned or state-controlled enterprises, and local authorities: to identify potential projects and initiate placing them in the budget cycle; and to conduct the initial feasibility study. The Government’s vision in adopting the PPP policy was to increase the pace of providing infrastructure using resources from the private sector. The rationale for the introduction of the policy was to speed up delivery, efficient delivery of projects, better maintenance of projects, cost savings, ease of capital budget planning, better provision services to customer, enable the public sector to concentrate on its core functions, and more efficient use of state-owned land. The Eswatini National Health Policy of 2016 has a theme of directing the health sector to aim for the attainment of Universal Health Coverage (UHC). It asserts through the policy statement on health financing that it “shall [be to] ensure financial risk protection for the population so as to attain UHC.” One of the policy implications is stated as “in collaboration with central agencies, the MoH shall plan, budget and explore alternative financing options to ensure equity and access to health services by all citizens”. It further underscores the importance of “…ensuring that efficient procurement mechanisms that guarantee value for money are adhered to”. In Eswatini, the identification and procurement of PPP projects in the health sector has been slow since 2008 and the involvement of the private sector and non-governmental organizations (NGOs) through the PPPs has been limited. There are a few PPPs being implemented in the health sector which do not go a long way in assisting the sector to expand the provision of quality services, in particular, tertiary services. Capacity of in-country private providers remains low and availability of skilled specialists in different fields of medicine remain a challenge. Thus, there seem to be opportunities for PPPs in the provision of tertiary services. The health sector has seen it important to formulate PPP guidelines to guide and fast track the implementation of the national PPP policy. These guidelines will give direction on how the PPP projects are to be identified, procured, and managed. Additionally, the guidelines will assist the health sector, inter alia, in the mobilization of more resources, reduced whole life costs, provision of affordable quality health services to both government and clients, increased Infrastructure development, faster implementation of projects, increased investment in the sector, and efficient use of resources. Furthermore, these guidelines should go beyond the traditional areas outlined above and assist in the efficient delivery of quality health services using PPP approaches. It is from this premise that a consultant needs to be hired to assist with the development of health-sector specific PPP guidelines, that are in line with the National Health Policy, the National Health Sector Strategic Plan, and existing laws.[1] • Objective of the Consultancy The main objective of the consultancy is to produce guidelines to aid the implementation of PPP in the health sector. 3. Eswatini COVID-19 Emergency Response Project (ERP) REQUEST FOR EXPRESSION OF INTEREST (CONSULTING SERVICES – FIRMS SELECTION) Title of Assignment: CONSULTANT TO Train Basic Public Health Emergency Management Reference Number: SZ-MOH-36081453918-CS-CQS In 2021, the Kingdom of Eswatini secured additional financing for Eswatini COVID-19 Emergency Response Project (ERP, P173883) from the World Bank to continue with the COVID-19 response and strengthen capacity in preparedness for health emergencies, including supporting vital institutional capacity-building efforts in the development of standardized frameworks and guidelines for health emergency preparedness and response. In addition to the current COVID-19 pandemic, Eswatini is under constant threat from other infectious diseases, droughts, storms, food contamination, antimicrobial resistance, traffic accidents and other emergencies. Despite the country’s commitment to strengthening health emergency preparedness and response capacity – a priority area, training, in the country’s National Health Sector Strategic Plan (2019-2023), remains limited. Eswatini scored low at 38 in its 2018 World Health Organization (WHO) Joint External Evaluation of the Internal Health Regulations (2005). Eswatini’s preparedness score was particularly low with no or little capacity. Eswatini’s Global Health Security Index score was also low at 31.1. Eswatini was classified by WHO as a ‘least prepared’ country. Areas for improvement in Eswatini included: • detection and response to emergencies within its overall health system capacity • a sufficiently robust health system to treat the sick and protect health workers, especially in times of public health emergencies. Specific capacity gaps were identified in:• hospitals and clinics • medical countermeasures • personnel deployment. In response, the Ministry of Health (MoH) is planning various activities to strengthen the country’s capacity in preparedness for health emergency including equipping key frontline health care workers with the training they need to ensure that they are prepared for and ready to respond public health emergencies. Purpose of the consultancy To provide key medical personnel assigned as Public Health Emergency Officer (PHEO), Medical Emergency Manager (MEM) and key frontline health workers in Eswatini with the knowledge and skills they need to ensure that they are prepared and ready to respond to public health emergencies by training them in the WHO Basic Public Health Emergency Management (PHEM) course. Requirements • The successful educational institution will have a faculty with experience in PHEM training. Plumbing Supervisor

HEALTH EMERGENCY PREPAREDNESS AND RESPONSE PROJECT JOB -ADVERTISEMENT REQUEST FOR EXPRESSIONS OF INTEREST Title of Assignment: Plumbing Supervisor Reference Number: SZ-MOH-358893-CS-INDV • Background of the Project The COVID-19 Emergency Response Project financed by the Bank will support the Kingdom of Eswatini’s effort to prevent, detect and respond to the threat posed by COVID-19 and strengthen national systems for public health preparedness. The Project Development Objective is to prevent, detect and respond to the threat posed by COVID-19 and strengthen national systems for public health preparedness in Eswatini. The project components are aligned with the objectives of the COVID-19 Strategic Preparedness and Response Program (SPRP) and comprise 2 components: (1) Emergency COVID-19 Response; and (2) Implementation Management and Monitoring and Evaluation. In addition, the Additional Financing (AF) expands Eswatini’s COVID-19 response through (a) co-financing vaccine procurement, (b) strengthening the systems required to support effective and efficient vaccine deployment; and (c) strengthening public structures for the coordination and management of the project under component 1 and adds another component to strengthen Capacity in Preparedness for Health Emergencies. A sub-project activity under the project includes the rehabilitation of drainage systems and plumbing works within 14 health care facilities in the country. These facilities include; Mbabane, Pigg’s Peak, Manzini, Mankayane, Hlathikhulu Government Hospitals, Lubombo Referral, Raleigh Fitkin Memorial Hospital, Good Shepherd Hospital and Nhlangano, Mkhuzweni, Dvokolwako, Sithobelweni, Matsanjeni Health Centres and Psychiatric Centre. Two of the facilities (Good Shepherd and Raleigh Fitkin Memorial Hospitals) are faith-based and are managed privately and they have their own plumbing and maintenance teams. Annex 1 details the scope of rehabilitation works that needed to be done at the facilities above. The activity is lead by BIOMED which is a department under MoH responsible for maintaining the Ministry’s equipment, electricity and drainage. BIOMED is supported by the Environmental Health Department (EHD). Both entities are responsible for the procurement of material, engagement and supervision of the rehabilitation works. Progress has been made on this activity. A total of 12 plumbers were engaged for 10 months starting from September 2021. Plumbing material was procured. However, the works were not completed due to the expiry of the plumber’s contracts. The Ministry of Health intends to re-engage the plumbers in July 2023 to complete the rehabilitation works. There is a need for a supervising agent to make an inventory of the rehabilitation works done in the facilities, assist in the development of a finalization program for the rehabilitation, re-engagement of plumbers, supervise the works to finalization. Below are the objectives and scope of work to be done by the Supervising Agent. • Objectives of the Consultancy The objective of the assignment is to assist BIOMED and EHD with supervision services of the plumbing rehabilitation works to be completed under the 14 health care facilities listed above. While progress was observed during the first phase of the rehabilitation, there were gaps in planning and programming of the works, tracking and supervision of the works. HEALTH SYSTEMS STRENGTHENING FOR HUMAN CAPITAL DEVELOPMENT PROJECT JOB -ADVERTISEMENT REQUEST FOR EXPRESSIONS OF INTEREST Title of Assignment: TECHNICAL ASSISTANCE (CONSULTANCY) TO DESIGN AND IMPLEMENT EFFICIENT PATIENT REFERRALS WITHIN THE HEALTH SYSTEM OF ESWATINI Reference Number: SZ-MOH-356309-CS-INDV • Background of the Project The Government of the Kingdom of Eswatini through the Ministry of Health (MoH) entered into a loan agreement with the World Bank effective 10 December 2020, to implement a Health System Strengthening for Human Capital Development Project in Eswatini (HSSHCDP). The project focuses on strengthening the health system and ramping up investments in reproductive, maternal, new-born, child, and adolescent health (RMNCAH), nutrition, and non-communicable disease (NCD) services to address critical human capital challenges, including stunting and child and adult mortality, applying a life course approach. The project has four components namely, Component 1: Improve health service delivery to increase the coverage and quality of health services to build human capital; Component 2. Increase community demand for RMNCAH, nutrition and NCD services; Component 3. Strengthen the MOH’s stewardship capacity to manage essential health and nutrition services and project activities; and Component 4: Contingent Emergency Response. • Background of the Assignment The Government of the Kingdom of Eswatini has defined the necessity to improve the current situation of the health system to prepare the health sector institutions towards the goal of universal health coverage. The process will involve a series of interventions which will lead to the creation of a resilient, efficient, effective, and equitable health system. The MOH has committed to strengthening health systems in all the thematic areas including clinical service provision through a timely, efficient, and effective referral system. Priority area three (3) of the current National Health Sector Strategic Plan (NHSSP 2019-2023) focuses on prompt and effective management of medical conditions, which supports the strengthening of patient referrals to improve retention in care. An effective referral system ensures continuity of care for clients with appropriate referrals (including upward referrals and down-referrals) in place to ensure service delivery along the continuum of care among all service providers. The Eswatini health system is based on a primary healthcare approach, organised into a broad three-tier system with five levels of service delivery: | 2 | 3 | 1 The MOH seeks to optimize the existing referral system to address challenges experienced over the years. These challenges include: failure to enforce the referral system nationally resulting in high patient volumes at Level 4 and 5 hospitals, in-effective and sub-optimal service delivery at the lower levels of care due to inadequate medicines and staff complement and observed poor linkage between emergency medical services and health facilities. The referral system will cover all five national health service delivery levels within the broad three-tier system. The design of the referral system will account for the characteristics of the Eswatini health system including referrals from community to health facility, within and between health facility, within and between regions, and between public and private sectors. An effective referral system was key in managing COVID-19 cases and lessons learnt from the response will be incorporated on this assignment emphasizing on pre-referral care, triage, and emergency care at the receiving facility. The Terms of Reference below seeks to identify a consultant to re-design and test an effective and efficient referral system that is equitable for all people residing in Eswatini. • Objective of the Consultancy The objectives of the assignment are: (a) to review the current patient referral system to identify gaps, barriers, constraints, strengths, opportunities, and threats through relevant methods; and (b) revise/modify/re-design the referral system to address the existing gaps and strengthen linkages across all levels of care and stakeholders. 4. Assignment Background In addition to COVID-19, Eswatini is under constant threat from other infectious diseases, droughts, storms, food contamination, antimicrobial resistance, traffic accidents and other emergencies. Despite the country’s commitment to strengthening health emergency preparedness and response capacity – a priority area in the country’s National Health Sector Strategic Plan (2019-2023), its capacity remains limited. Eswatini scored low at 38 in its most recent (2018) World Health Organization Joint External Evaluation of the Internal Health Regulations (2005). Eswatini’s preparedness score was particularly low with no or little capacity. Eswatini’s Global Health Security Index score was also low at 31.1. Eswatini was classified as a ‘least prepared’ country. Areas for improvement in Eswatini include: • detection and response to emergencies within its overall health system capacity • a sufficiently robust health system to treat the sick and protect health workers, especially in times of emergency. specific capacity gaps were identified in:• hospitals and clinics • medical countermeasures • personnel deployment • healthcare access • communications with healthcare workers during a public health emergency • infection control practices • availability of equipment, and • capacity to test and approve new medical countermeasures. In response, the Ministry of Health (MoH) is planning to carry out various activities to strengthen the country’s capacity in preparedness for health emergency by developing National Health Simulation Exercise, and Emergency Preparedness and Response Strategy, and conducting a health assessment for priority health conditions. Objective of the consultancy In order to ensure smooth implementation, the MoH is recruiting a public health consultant to support the MoH in building capacity in health emergency preparedness. Health financing, as one of the policy directions, places emphasis on the need to explore financing options that will ensure equitable access to quality health services by all while providing financial risk protection, towards attainment of UHC. The National Health Sector Strategic Plan (NHSSP) 2019-2023 anticipated epidemiological transition of the country, growth in population and life expectancy, and an increase in the health needs of adolescents, youth and the elderly, particularly with regard to Non-Communicable Diseases (NCDs). In addition, a decrease in external aid in nominal terms had become evident and was anticipated to continue, hence the need to build secure and resilient health systems. The strategic direction and principles set in the NHSSP 2019-2023 attest to MOH commitment to attaining Universal Health Coverage (UHC), which has been threatened by among other issues resource constraints, and this was significantly tested by the emergence of the Covid 19 pandemic. In 2018, the Ministry of Health (MOH) embarked on a process of health reforms among which was the development of a comprehensive National Healthcare Financing Policy (NHFP). This policy outlines aspects of health care financing models. The Vision of the policy is ‘Affordable health care and protection from catastrophic expenditure for all Swazis’. The policy proposes thematic areas such; 1) Sustainable Healthcare Financing, 2) Financial Risk Protection for all Swazi Nationals, 3) Efficiency in resource allocation and resource use; and 4) Equitable Access to Healthcare Services. It is therefore against this brief background that the Ministry of Health seeks Technical Assistance (TA), to develop a National Healthcare Financing Strategy (NHFS) with a clear strategic intervention implementing the NHFP aspirations. • OBJECTIVES OF THE CONSULTANCY The Consultancy is required to assist the Ministry of Health to develop a Health Financing Strategy that will enable the implementation of the National Health Financing Policy strategic objectives, which are: • Mobilizing more resources for health in an equitable and sustainable manner • Strengthening financial risk protection measures for all Swazi nationals, especially for the poor and vulnerable • Enhancing efficiency and equity in resource allocation and use 3. QUALIFICATIONS AND EXPERIENCE • At least a Master’s degree in Social Sciences, Health Economics, Development Studies, Public Health or other relevant field. • Minimum 7 years working experience in the health financing field, strategic plan developments, costing of strategies and interventions, efficiency analysis with at least three (3) years in the Sub-Saharan Africa region. • Specific experience in developing a health financing strategy in a similar context • Experience as team leader of projects providing policy advice and capacity building including facilitating peer-to peer exchanges among civil servants. • Strong analytical skills, excellent team and leadership skills, strong interpersonal and communication skills are a necessity. • Computer proficiency with high level of familiarity with commonly used and advanced Microsoft software • Demonstrated ability to work in multi-disciplinary team environment • Be proficient in English language, written and spoken • TIME FRAME OF ASSIGNMENT AND PAYMENTSCHEDULE The duration of the assignment is 120 working days over a twelve-months period. 5. IMPLEMENTATION ARRANGEMENTS MOH will be the executing agency. The Principal Planning Officer in the Planning Unit will be directly responsible for the day-to-day implementation of this consultancy. An individual consultant will coordinate all the activities in this consultancy. A health financing technical working group (TWG) is available to provide overall guidance to the consultancy and ensure coordination with Ministries and agencies concerned. The consultants will present progress of his/her activities to the TWG at regular intervals preferably monthly and seek feedback and guidance. The consultant will be solely responsible for submitting progress briefs and reports outlined above. 6.
