REQUEST FOR CURRICULUM VITAE FORINDIVIDUAL CONSULTANT TO PERFORM AN ASSESSMENT OF THE HEALTH CARE DELIVERY SYSTEM IN PREPARATION FOR THE IMPLEMENTATION OF THE HEALTH CARE PACKAGE (EHCP) IN SWAZILAND

Background Information

The Governmentof Swaziland has received financing from the International Bank for Reconstruction and Development (IBRD) towards the cost of a five-year Health HIV/AIDS and TB Project in the amount of US$20 Million equivalent. This project will be jointly financed by the European Union in the amount of Euro 14.5 million.

The project has three components: the first two, (i) strengthening the capacity of the health sector and (ii) facility-level support to improve access, quality and efficiency of services will be implemented by the Ministry of Health (MOH) and the third, (iii) strengthening of the orphans and vulnerable children safety net will be implemented by the DPM's Office.

The Health, HIV/AIDS and TB Project, has made provision for Individual Consultant toperform an assessment of the Health Care Delivery System. The main objective of the consultancy is to assess the current Public Health CareDelivery Network in order to prepare the implementation of EHCP launched in February 2012

The MOH now invites interested, qualified candidates to indicate their interest in providing technical support and services under this consultancy. The duration of this Consultancy is sixty five (65) days spread out in five months. Selection shall be on the basis of examination and comparison of the qualifications of the individual candidates as evidenced by their CVs. The selected individualshall be requested to submit a costed offer to undertake the work.

Specific Objectives for the Baseline Assessment:

a) Assess the health care services defined in the EHCP. For each essential service, availability of key aspects. As defined in the official document of EHCP Ministry of Health of Swaziland Including but not limited to:

  • Family Health Services
  • Prevention, Management and Control of Communicable Diseases

Prevention, Management and Controlof Non-Communicable Diseases

Health Promotion Services

  • Essential Clinical Care Services
  • InternalMedical Clinical Services
  • SurgicalServices
  • Allied Health Services
  • Support Services

b) Identify gaps in resources at facility level that could be barriers to offering the services defined in the EHCP e.g. staffing, infrastructure, equipment and commodities, according to standards defined by the MOH that are required to implement the EHCP in Swaziland.

c) Determine the areas where interventions (resource mobilization, training, mentoring or supervision) will be required to facilitate the implementation of the EHCP.

d) Draw specific lessons of relevance for the country regarding the implementation and potential adjustments of the original EHCP

e) To estimate the cost of the EHCP as currently is defined by the MOH/WHO proposed package of essential services for Swaziland by level of care, in a convenience sample of 17 health facilities, such as Community Health Clinics, Public health Units, Health Centers,Regional Hospitals and One National Hospital to be selected by the MOH authorities (four health facilities per region and one National Hospital).

f) Each health facility assessed will constitute a case-model to determine gaps, needs, and to define the actual cost of the current EHCP, and will include also suggestions or proposals from the consultant in order to adjust the current EHCP to the conditionsof those facilities and considering the fiscal limitations of the country.

g) To develop an Implementation Plan

### Specific Activities.

The consultants are required to develop the following activities:

a) Review key literature on the nature, scope, objectives and approach of the EHCP

b) Review international literature for specific countries (mainly Sub-Saharan Africa) and extract lessons about way they adjust and implement their EHCP

c) Develop the proper instruments/tools to collect the required information, which will be previously approved by the MOH.

d) Based on the results of the assessment, prepare an estimation of the required (gap analysis) human resources, physical andequipment resources required in Swaziland to adequately implement theEHCP and calculation of the corresponding gaps in the convenience sample of facilities (17 in total as described in paragraph (e) of the specific objectives. And prepare 17 specific cases (four for each Health Region including one Community Health Clinic, one Public Health Unit, one Health Center, one Regional Hospital, and one National Hospital) which will include:

  • Current status (accessibility, Human Resources for Health, Technology, MedicalSupply, clinical management tools, and general status of buildings).
  • Gaps definition and required interventions in each area (as described in item (a).
  • Proposedadjustment to the Current MOH/WHO proposed EHCP.
  • Conclusions and recommendations.
  • Estimation of the marginal cost for the country to implement the EHCP (both, the adjusted and Non-Adjusted).
  • Definitionof major guidelines for the implementation of the recommended EHCP adjusted) (Implementation plan)

Qualifications:

The consultancy requires an Individual Consultant who must fulfill the following qualifications and experience:

a) Economist with a Master's degree in health Economics or equivalent, Public Health, and/or Health Management.

b) Biomedical Engineer or equivalent with a Master degree in health facilities planning or equivalent.

c) Must have at least 5 years of professional experience in the health care sector, especially at international level and in developing countries, with considerable experience in the implementation of public health and health financing projects.

d) Must have proven experience of not less than 5 projects in one or more of the following topics:

  • Definition, improvement or adjustments of benefits packages in health care,
  • Costing of health interventions and investments,
  • Financing of the health sector and,
  • Health care facilities planning

e) Must have proven experience working (not less than 3 experiences during the last 7 years) in developing countries. Previous experience in Africa is an advantage
